Process for assistance to loan medical equipment for home care use

Patient must be at home. ( Not in a paying facility)
Responsible person staying with the patient must make contact for assistance.
Once clarity is obtained, arrangements must be made to collect from us during business hours.
Responsible person staying with the patient must come themselves.
Need to bring photo copies of ID of both the patient and responsible person.
Above refers to Wheelchairs etc besides Hospital beds.
Hospital beds will be delivered by us within a 60km radius.
All deliveries are done during the evening as our volunteers are at work.
Excluding weekends and public holidays.
